Payment Information:
Payment is accepted in the forms of cash, check, or credit/debit card. Checks must be personal checks presented with
drivers license and will be processed electronically. There will be a $25.00 charge for returned checks. Failure to render
payment may result in collection action. In selected cases, you may set up a payment plan via DocPay.
Insurance Patients:
Regardless of your insurance coverage, you are responsible for payment for non-covered services, co-payments and deductibles.
Please make sure to check if you need pre-authorization or a referral prior to your visit. You may want to check with your
insurer to see if you have out-of-network benefits, which may reimburse you for part of your expenses. You will be responsible
for payment at the time of service and your bill can be submitted to your insurance, which will decide whether or not to cover
an out-of-network claim.
