POLICIES AND PROCEDURES
Medical Information:
For your first appointment, it would be helpful for you to bring previous medical records, evaluations, medications, laboratory tests, and/or results of drug screens if available. If you need detoxification, please call so I can direct you to the appropriate facility.

Financial Policy:
Payment is expected at the time of service in the form of cash, check, and credit cards unless other arrangements have been made. A $25.00 fee applies to all returned checks. Check draft payment plans can be arranged for an additional fee.

Medical Insurance
At this time we are not accepting any medical insurance plans. Payment is due at the time of service. If you want to be reimbursed by your insurance company, we will assist you in filing your claim. Please remember to bring your current insurance card and picture ID. This will help us communicate with your insurance company if needed.   Please remember that you are responsible for any deductibles, co-payments, and non-covered services.  Medicaid nor Medicare are not accepted at this office.

You will be supplied with the information necessary to assist you in filing a claim. Your reimbursement may not cover the full cost of your studies or physician services. Regardless of insurance, payment remains your personal responsibility.

Cancellations:
Patients are seen by appointment only. If you are unable to keep your appointment, please notify us as early as possible, so that we’ll be able to offer the time to another patient.

E-mail communication:
Because e-mail is not a secure communication tool, you may want to keep your inquiries general in nature.

We look forward to meeting you and assisting you in your recovery.
